Kacchan hasn’t looked happy, but he’s never had to be on bedrest before.
Izuku wants to reach out and...what? Swing his arm around him, like Iida occasionally will be willing to do to Izuku? Hug him, like Ochako does when she sees Izuku frowning? Nudge his shoulder, like Izuku does to Todoroki?

Taking care of others is tough work. More often than not, you end up with a few bruises for your efforts, but that’s okay.
They heal and you move on. But it’s not as easy when they don’t fully go away before the next battle.
The bruises start to stack, and the healing never fully happens, and you start to ignore your own healing…since it’s not like you can avoid your duty.(or: Difficult conversation happens after Izuku comes home late from work yet *again*)
